I hereby authorize WYES-TV to initiate debit entries to my account indicated on the electronic form. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law.
This authorization is to remain in full force and effect until WYES-TV has received written notification from me of its termination in such time and in such manner as to afford WYES-TV a reasonable opportunity to act on it.
Furthermore, if any such electronic debit(s) should be returned by my financial institution as Non-Sufficient Funds ( NSF ), I authorize WYES-TV to collect a returned item fee up to the amount required to process the return item by electronic debit from my account identified above.
The recurring EFT payment will be withdrawn from my bank account and will be updated there on within one business day of the 5th of each month.
I understand and authorize all of the above as evidenced by my signature on the appropriate electronic form.
If I choose, I may also fill out a paper form, and mail it to the station to include my signature on file. If a digital signature is not included I will download the form, and send it to the station so that this process can be completed in a timely manner.
